I just completed a full run on Advanced "without losing a ship" as required for this specific achievement and it did not unlock (I was using Tee, although this shouldn't matter). I have a replay saved and verified that I did not lose any lives throughout the entirety of the run. This was quite disappointing to say the least as this took quite a bit of time to practice for.
