It's like any other roguelike game. It's pretty difficult from the start since you will lack the game knowledge, but once you get the feel for it, boy does the gameplay deliver (speaking from Isaac experience).
I suppose what's really good about the game is it's sound design and simple gameplay that rewards the player by not trying out the same strategy over and over again.
If you struggle with understanding the game, I recommend Balatro Basics 1 from Balatro University. He covers the basic strategy and how to think when building your run. It's definitely worth a try and got me hooked.
