As a hardcore gamer craving difficulty, I can tell you this: Doom Eternal in Nightmare mode is hard. Really hard.

The game is not unfair, but it will feel like it if you are not trully committed. You will die, a lot, but the game is optimized and designed to let you retry really fast, so it's not frustrating.

The timings, knownledge and the APM required to survive are really insane. So if you were missing a FPS that takes the player seriously like none did it in ages, just go for it.
But be ready to understand that you may actually not be that skilled of a gamer :)

Small addition: everyone is saying how ammos are sparse and how you need to be careful with them. That's lies.
You have ammos fully restored with one use of the chainsaw, and the chainsaw is a mid-range one-shot freekill that gives you temporary invulnerability with a cooldown. If you are any smart, you will exploit it so much you cannot possibly run out of ammos.
Doom Eternal is played by keeping that left mouse button pressed at all time.
