the second loadout sounds like good idea but I've got alot of expirience as medic and i know that that there are very few situations you can actually use stuff like the overdose & amputator and hope to secure a win, honestly i laugh whenever i see someone using them cause they're just that bad and if you think they're viable then that's just fine but it really doesn't belong in a guide, I'm not saying you've gotta remove it but maybe you should list it as a very niche possiblility rather than an alternative set

Solemn Vow
I use medigun about 60% and crossbow 40% because i am very good at hitting bolts.
Solemn Vow lets me know if the situation calls for fight or flight.
I personally prefer to overheal (in this order) Sniper, Spy, Medic, Engie, then everyone else.
A tip to keep in mind is do not use Quick-Fix uber like a regular uber. Use it when a lot of your teammates are low and taking fire and juggle their hp.
