What I ended up doing for my first play through is once I unlock a stage I run through on normal WITHOUT starting any main mission, and slaughter everything as fast as possible, THEN I go back and play the story missions on Manic difficulty - it's been a heck of a lot more enjoyable imo... normal is just way, way too easy, especially if you use the weapons and armor the game give you for existing.
...that said, you still can't play on Manic difficulty if it's a mission where you're accompanied by an AI teammate.
I had to look it up online to find out wtf was going on - it might not be the most interesting plot at the beginning, but overall this game has a pretty good story and great characters, so it sucks that I'm 100% sure so many new players don't have a clue what's going on.
