This is the ultimate ending with extra scene, after playing this game i figured that there are lots of possibilities about it, what i've thought of:
I wanted to think that Aya didn't became crazy and started killing people like her father, she just opened a clinic and started treating people for free (such a good girl), I say this because the red book she found is called "The New Anatomy Book" (it was her father's) she could have learnt from it.
The other possibility is that she actually became crazy, you could think of this because Jean was brought by an old woman (Maria), Aya says she likes her eyes (for a doll), there's a ladder to the basement right next to Maria and there are like three dolls in the clinic. It is sad I know, but i want to believe the first one, haha.

About the extra scene, Ogre is a demon and brought Aya's father back to life into the underworld and he made a clone of Aya. At least that's what i can say with a little proof.

Oh Sorry about the Skype call sound and about going too fast with the dialogue, silly me :)
